The NHTSA found that in 2,046,000 of 2,189,000 car accidents, "the critical reason (was) attributed to drivers." That comprises "94 percent of the crashes at the national level." 

Those human errors were further classified by the NHTSA into four classifications: "Recognition Error," "Decision Error," "Performance Error," and "Non-Performance Error." 

"Recognition Errors" were defined as including the "driver's inattention, internal and external distractions, and inadequate surveillance." "Decision Errors" included "driving too fast for conditions, too fast for the curve, false assumption of others' actions, illegal maneuvers, and misjudgment of gap or others' speed." 

"Overcompensation, poor direction control, etc." made up "Performance Errors," while "Non-Performance Errors" was mostly defined as the driver falling asleep. 

"These disturbing findings from the National Highway Traffic Safety Administration show just how many responsibilities we have when we're behind the wheel of our cars. You have to pay attention to road conditions, eliminate distractions to the best of your ability inside and outside the vehicle, while properly judging the speed of your vehicle and others, managing your speed, and following the rules, and, of course, making certain that you're awake enough to be behind the wheel.
